Most likely causes, in order

Warm, humid house air leaking into the attic through the hatch, recessed lights, top plates and pipe penetrations, then condensing on cold sheathing in winter.
A bathroom or kitchen exhaust fan venting into the attic instead of through the roof or wall.
Soffit vents blocked by insulation, so the attic cannot flush the moisture out.
Too little insulation, keeping the sheathing colder and the attic warmer and damper than it should be.
A roof leak, usually at a flashing or a valley, which shows as a localised patch rather than widespread staining.

Why the price varies so much

Access and area. A walk-in attic with staining on one slope is a day of work. A low, tight attic with staining across the whole underside of the roof, blown insulation to work around, and a steep pitch is several days, and the crew is on their knees for all of it. The method matters too: a wipe-down and HEPA vacuum of light staining costs far less than media blasting heavy growth off every rafter bay.

The part most quotes leave out

Attic mold is a symptom. In almost every attic we see, the moisture comes from the house below: a bathroom fan blowing into the attic, warm air leaking up through the hatch and light fixtures and condensing on cold plywood in winter, and soffit vents buried under insulation so nothing flushes out. Clean the sheathing without fixing those and the staining is back within a winter or two, and you pay twice. A good quote lists the cause fixes; a bad one only lists the cleaning.

When to call a professional

Staining covers more than a few square feet, or the sheathing is soft, delaminating, or sagging.
You cannot find where the moisture is coming from, or the fixes above have been done and it came back.
You want an independent scope before paying a remediation quote. A written scope from someone who does not do the cleaning keeps the job honest.

What not to do

Do not pay to fog or spray the attic and call it done. Fogging does not remove growth or fix moisture.
Do not let anyone paint or encapsulate sheathing that is still damp. Check it with a moisture meter ($25 to $40); framing and sheathing should read below 15 percent before anything goes over it.
Do not add more roof vents without opening the soffits. Exhaust with no intake pulls conditioned air out of the house and can make the moisture worse.
